Suspensory ligament
The suspensory ligament is an important anatomical structure in the male body. It plays a crucial role in stabilizing the penis and is particularly relevant in connection with penis lengthening through the ligamentotomy. At our center, we work intensively with this ligament, as it is of central importance for surgical procedures in intimate surgery.
What is the suspensory ligament?
The suspensory ligament (Latin: ligamentum suspensorium penis) is a fibrous ligament that connects the penis to the pubic bone (technical term: Os pubis) connects. It acts like a suspension that fixes the penis to the body and gives it support.
This ligament ensures that the penis points outwards at a certain angle – both when flaccid and erect:
- It originates in the area of the anterior pelvis, i.e. on the bone above the penis.
- It runs downwards from there and merges into the tissue that supports the penis inside the body.
- It forms a kind of "suspension" for the penis, especially when flaccid.
This ligament consists of taut connective tissue that is very firm but slightly elastic.
Anatomical position
- The ligament extends downwards from the pubic bone (symphysis) and connects to the base of the penis.
- It lies under the skin and is not visible to the naked eye.
- Together with the deeper fundiform ligament, it stabilizes the penis.
Task and function of the suspensory ligament
The suspensory ligament fulfills several important functions:
1. stabilization of the penis
In everyday life and during movements – for example when walking, sitting or doing sport – the band keeps the penis in a stable position. It acts like a kind of “belt” that holds the penis to the body.
2. erection support
During an erection (i.e. when the penis becomes erect), the ligament helps to keep the penis in its erect position. It is not directly responsible for the erection – that is done by other mechanisms in the penis itself – but it contributes to stability.
Significance in surgery
The suspensory ligament plays a major role in intimate surgery:
- This ligament is severed during a ligamentotomy.
- The part of the penis that is inside the body is moved to the outside by cutting it.
- The result: the penis looks longer, especially when flaccid.
Without the ligament, the penis would be less stable and would hang down more. Careful fixation and aftercare is therefore very important after a ligamentotomy to ensure both length and stability.
Combination with other methods
- Ligamentotomy: lengthening by cutting the ligament.
- Lipofilling (autologous fat tissue transplantation): Thickening of the penis by injecting autologous fat.
- Both methods together can optimize length and circumference and thus create a balanced result.
Advantages of targeted surgery on the ligament
- Visible lengthening: when flaccid - often also when erect
- Combinability: Enhances the effect of other procedures such as lipofilling (penis thickening).
Risks and limits
- Change in the erection angle: After cutting the ligament, the penis can point slightly flatter downwards when erect.
- Risk of recurrence: The ligament can grow back together without good fixation or aftercare.
- Need for aftercare: To stabilize the new length, it is often necessary to use an extension device.
The suspensory ligament is the anatomical basis for many penis lengthening procedures. It is important to ensure a precise surgical technique and consistent aftercare so that the benefits of a ligamentotomy are preserved as well as possible. In combination with lipofilling (penis thickening) or other methods, a functionally convincing result can be achieved.
